【論文名】Pembrolizumab plus chemotherapy versus chemotherapy alone in patients with advanced non-small cell lung cancer without tumor PD-L1 expression: a pooled analysis of 3 randomized controlled trials 【出典】Cancer 2020年9月11日オンライン版 連載第33回に選んだのはCancer誌から「Pembrolizumab plus chemotherapy versus chemotherapy alone in patients with advanced non-small cell lung cancer without tumor PD-L1 expression: a pooled analysis of 3 randomized controlled trials」です。 進行非小細胞肺がん（NSCLC）の一次治療では現在、非扁平上皮がんでも扁平上皮がんでもPD-L1発現率（Tumor Proportion Score；TPS）にかかわらず、免疫チェックポイント阻害薬とプラチナ製剤併用化学療法の複合免疫療法が適応になります。ただし、PD-L1の発現率が高い方が治療効果が期待できるため、実臨床ではPD-L1 TPSも治療選択の参考にされているものと思います。 今回、NSCLCに対する3件のぺムブロリズマブを含む"ケモコンボ"の第Ⅲ相試験の統合解析から、PD-L1発現陰性症例に対する効果を見た報告がありましたので勉強しました。
